Just in case anybody else out there wants the info, here you go:

There is a form which your chapter secretary/president/whomever keeps the
forms should have which can be filled out and sent to the office, along
with the required $40.00.  In the event that said form cannot be located,
you may call 816-373-8667 and Judy Mitchell will assist you.  Be aware -
you must know who the recipient is before you call, since the Office is
now tracking DSK recipients and won't send you one without knowing to whom
it is being awarded.  Anyway, Judy will ask the appropriate questions to
fill the form out, but you still need to send the check to the office.
The office will overnight (read "Federal Express") the DSK for an
additional charge, but if one plans ahead, this won't be necessary.
Usually, the office turns around requests for all materials which are in
stock within a week or so.

I have ec'ed Pat Burke - so if any of this has changed, I'll ask Pat to
correct me.
